NEW PATIENTS:
Before your appointment, please complete all intake paperwork and consent forms sent to you after scheduling your treatment.
INSURANCE:
We value simplicity and accessible healthcare. Debit, credit, HSA/FSA, check and exact cash is accepted at Sacred. We are an Out-of-Network Provider and do not accept insurance, which still leaves you the option to self-submit to your insurance if you opt to. We do not accept personal injury or medicare/medicaid, but are happy to make a referral to a practice if needed.
